Formal Participant Agreement
Waiver of Liability & Cancelation Policy
I agree to the following:

1. That I am voluntarily participating in the Feminine Wholeness Retreat (July 21 - July 27, 2024) in Greece offered by Maria Lentzou and Authentic Movement Soul Freedom (the Provider) during which I will receive instruction about Authentic Movement, yoga, expressive arts and health. I recognize that any movement practice may cause physical injury. I am fully aware of the risks and hazards involved and participate at my own risk.

2. I am responsible for consulting with a physician, psychotherapist, or other medical professional before attending the Feminine Wholeness Retreat regarding my suitability to participate. I represent and warrant that I have no medical condition that would prevent my full participation in the retreat experience.

3. I will assume full responsibility for any risks, injuries, or damages, known or unknown, that I might incur due to participating in the Feminine Wholeness Retreat program. This includes, without limitation, my use of the facility and any pool, whirlpool, sauna, parking area, sidewalk or any equipment in the facility and my participation in any activity, class, program or instruction.

4. I agree that I am voluntarily participating in the Feminine Wholeness Retreat and using the facilities and premises and assume all risk of injury or death to me or the contraction of any illness or medical condition that might result, or any damage, loss, or theft of any personal property. I agree on behalf of myself (and my personal representatives, heirs, executors, administrators, agents, and assigns) to RELEASE AND DISCHARGE Maria Lentzou, the Authentic Movement Soul Freedom Staff, and/or Authentic Movement Soul Freedom (and their affiliates, employees, agents, representatives, successors, and assigns) from any and all claims or causes of action (known or unknown) arising out of my participation in the Feminine Wholeness Retreat (including, but not limited to our negligence). This Release and Waiver of liability includes, without limitation, injuries that may occur as a result of (a) my use of any exercise equipment or facilities that may malfunction or break, (b) our improper maintenance of any exercise equipment or facilities, (c) our negligent instruction or supervision, (d) my slipping and falling while in the facility or on the premises, and (e) my travel to or participation in the Feminine Wholeness Retreat. I acknowledge that I have carefully read this Release and Waiver of Liability and fully understand that it is a release of liability. I am waiving any right that I may have to bring legal action to assert a claim against us for our negligence.

5. I consent to receive medical treatment which may be deemed necessary in the event of any illness, accident, injury, or medical emergency resulting from or in connection with my participation in the Retreat and understand that I am solely responsible for all costs and expenses related to such medical treatment, medical transportation, and/or evacuation. This responsibility includes, but is not limited to, medical costs, hospital bills, and all other expenses related to healthcare services. You acknowledge that any injuries you sustain may be compounded by the Provider's negligent emergency response or rescue operations.

6. I agree that neither I nor any person or entity acting on my behalf has or will file, charge, claim, sue, or permit to be filed, charged, or claimed any action for legal or equitable relief against Maria Lentzou, the Authentic Movement Soul Freedom Staff, and/or Authentic Movement Soul Freedom (and their affiliates, employees, agents, representatives, successors, and assigns).

7. I have read and understand the Travel Information and cancellation policy below.

Acceptance of Terms by Booking:
By booking your space for the Feminine Wholeness Retreat, you acknowledge and agree that such booking constitutes your acceptance of the terms and conditions outlined in this Agreement, functioning as your digital or electronic signature. Your booking and participation in the Retreat signify that you have read, understand, and agree to be bound by the terms of this Agreement, including any and all liability waivers, indemnity clauses, and media usage policies herein.

Travel Information & Cancellation Policy
Travel to Destination Country:
You are responsible for securing and paying for any transportation costs to the Destination Country. The Provider bears no responsibility or liability for the delay or cancellation of the transportation you arrange. Transfers to/from the Retreat location are at your own cost unless otherwise specified on the retreat/event page.
Cancellation by Participant:
The deposit (applied towards the Retreat) is non-refundable and non-transferable (the “Deposit”). If you cancel 90 days or more before the Start Date, you will receive a refund less the Deposit. There will be no refunds, no transfers, and no credits for cancellations within 90 days of the Start Date; provided that, a refund less the Deposit will be provided to you to the extent that another person replaces you in the Retreat and pays the Deposit to the Provider. There will be no refunds or credits for your failure to attend or complete the Retreat. If you cannot travel to the Destination Country due to unforeseen travel restrictions beyond your control, the Provider hereby agrees to provide you with a refund less the Deposit.
Cancellation by the Provider:
The Provider reserves the right to make any necessary changes, including, but not limited to, advertised excursions, arranged transportation, and advertised meals. If the Provider cancels the Retreat (the “Cancellation”), the Provider will offer a full refund of your payment, including the Deposit. The Provider is not responsible for any expenses or damages you may have incurred due to the Cancellation including, but not limited to, your preparation costs, airline tickets, travel documents or other expenses.
Travel Documents:
You are responsible for learning about and obtaining all required travel documents, medical examinations, and vaccinations for travel to the Destination Country. The Provider is not responsible for any delays, cancellations, or damages due to my lack of proper travel documents, medical examinations, quarantine compliance, or vaccinations, including a valid passport, visas (entry and exit), health certificates, and other documents that may be required by the Destination Country or the country to which you will be returning.
Media, Indemnity & Miscellaneous
Media:
Media is defined as photography, video, written or verbal testimonial, or any other form of capturing likeness. You grant to the Provider an irrevocable, non-exclusive, fully paid-up, royalty-free, worldwide right and license concerning any media that the Provider has taken or may take of you during the Feminine Wholeness Retreat to record, publish, distribute, incorporate into other material, and otherwise use your name, image, words, recorded actions, and likeness, in whole or in part, for the Provider’s current or prospective business purposes, including in any promotions, publicity, advertising, marketing, and recruiting. If you wish to opt out of any or all such uses, you must notify the Provider in writing before the commencement of the Retreat, specifying the types of media usage you are opting out of. You agree that the Provider will be the exclusive owner of all copyrights and other rights in and to all tangible works and any results from using your name, image, words, recorded actions, and likeness.
Indemnity:
You will defend, indemnify, hold harmless, and reimburse the Provider from and for all damages, losses, costs, or expenses (including attorneys’ fees) incurred by the Provider or paid by the Provider to any person (including you or your insurers) in respect of any accident, injury (including death), loss, or property damage, resulting from, arising out of, or otherwise in connection with your participation in the Retreat. You will reimburse the Provider if anyone makes a claim against the Provider concerning your participation in the Retreat, including any accident you may be involved in or any injury, loss, or damage to me, other parties, or property. You further agree to indemnify the Provider from and against any liabilities, losses, claims, demands, costs (including reasonable attorneys’ fees), and expenses arising in connection with any breach or alleged breach by you of any of your representations and warranties in this agreement.
Miscellaneous:
These Event Policies constitute the sole and entire agreement you have entered into concerning the subject matter contained herein and supersedes all prior and contemporaneous understandings, agreements, representations, and warranties, both written and oral, concerning such subject matter. If any term or provision of the Event Policies is held invalid, illegal, or unenforceable in any jurisdiction, such invalidity, illegality, or unenforceability shall not affect any other term or provision of this Agreement or invalidate or render unenforceable such term or provision in any other jurisdiction. The Provider may transfer, assign, or sublicense their rights under this agreement or all or any of their rights or privileges hereunder without restriction. This Agreement is binding on and shall inure to the benefit of the Provider and you and your respective heirs, successors, and assigns. All matters arising out of or relating to this Agreement shall be governed by and construed in accordance with the laws of the State of California without giving effect to any choice or conflict of law provision or rule (whether of the State of California or any other jurisdiction). Any claim or cause of action arising under this Agreement may be brought only in the federal and state courts located in San Francisco, California, and I hereby consent to the exclusive jurisdiction of such courts.
